Willow Creek Healthcare Center held the position of the third largest nursing home in Fresno County during the first quarter of 2026, based on data from the Centers for Medicare and Medicaid Services (CMS).
The facility offered 159 available beds, surpassing the Fresno County average by 66 beds during the period. On average, it provided care for 151.7 residents in the quarter.
The CMS awarded Willow Creek Healthcare Center an overall rating of 3 out of 5, trailing the California state average rating of 3.2 for nursing homes.
In the same period, Pacific Gardens Nursing and Rehabilitation Center emerged as the largest nursing home in Fresno County with 188 beds, followed by Horizon Health & Subacute Center in second place with 180 beds.
By the end of the first quarter of 2026, Willow Creek Healthcare Center had incurred $25,045 in fines over two incidents and received three penalties.
There were a total of 24 for-profit nursing homes operating in Fresno County, California.
Estimates from the U.S. Census Bureau report that the nation’s senior population continues to rise as numbers in younger age groups decline. From 2020 to 2024, Americans age 65 and over increased by 13%, while the under-18 population declined by 1.7%. The growth in the older adult population has accelerated significantly over the last century, with an increase from 4.9 million in 1920 to over 61 million by 2024.
A 2021 analysis indicates that nearly 50% of adults in this age segment, or approximately 28 million people, will require long-term services and support. The research also predicts that more than one-third of these individuals may eventually need care in a nursing home.
“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance, an insurance education group. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
