During the first quarter of 2026, Horizon Health & Subacute Center ranked as the second largest nursing facility in Fresno County, the Centers for Medicare and Medicaid Services (CMS) reported.

The facility offered 180 beds—87 more than the county average as of the first quarter of 2026—and served a typical census of 153.3 residents during the period.

CMS assigned Horizon Health & Subacute Center an overall rating of 2 out of 5, trailing the California nursing home average of 3.2.

In that quarter, Pacific Gardens Nursing and Rehabilitation Center led Fresno County in size with 188 available beds. Willow Creek Healthcare Center followed Horizon Health & Subacute Center with 159 beds.

As of the first quarter of 2026, Horizon Health & Subacute Center had incurred $22,913 in fines and faced one penalty.

There were 24 for-profit nursing homes operating throughout Fresno County, California.

Estimates from the U.S. Census Bureau indicate the nation’s older adult population is growing while younger age groups contract. Between 2020 and 2024, Americans age 65 and up increased by 13%, whereas those under 18 declined by 1.7%. Over the past century, the country’s older adult population has surged from 4.9 million in 1920 to more than 61 million as of 2024.

A 2021 study indicates that nearly half of this group—close to 28 million people—could require long-term services and support, with more than a third expected to need nursing home care at some point.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
